Patients are vulnerable to mistakes made in any one of the many steps involved in the ordering, dispensing and admin istration of medications. Medication errors are one of the most common errors across all medical specialties. Accuracy requires that all steps are correctly executed. Common medi cation er rors include: /uni25CF poor assessment or inadequate knowledge of patients and their clinical conditions; /uni25CF inadequate knowledge of the medications; /uni25CF dosage calculation errors; /uni25CF illegible handwriting; /uni25CF confusion regarding the name or the mixing up medica tions.
